So you've chosen Paradigm. Now What?

Below we provide you with all the information, tools, and items you should need to make your first visit (and all your other visits) as comfortable and easy as possible. Look through the following guide before your first visit for some tips and points on what to expect, what you'll need to bring along with you, and even what you might need to wear to make your visit go smoothly.
What to Expect

First Visit: Your first visit to Paradigm will be an information collecting process and will include your initial evaluation with a physical therapist. In order to speed up the information collection process, we provide a downloadable New Patient Packet of paperwork you can fill in before you arrive for your first visit. Filling in as much of this information packet before your first visit will speed along the process of your first visit and cut down on your waiting time. (Normally this packet would have to be filled in at our reception desk on the first visit.) If you have questions about the paperwork or can not completely fill in the form, fill in as much as you can and ask questions when you arrive at our front desk. Please fill in the forms to the best of your ability, and don't worry if they aren't complete. We can help you finish when you arrive.

Steps to Success

     • doctors prescription
     • insurance card / information
     • new patient packet
     • list of your current medications
4. Wear comfortable, loose fitting clothing
5. Budget up to 2 hours for your first visit
Things You Need To Bring

When you arrive for your first visit, please check in with the front reception desk and provide us with a few items we need to complete your file. To begin the physical therapy process, we will need the following:

  • a valid doctors prescription (usually provided on your most recent doctor visit)
  • your insurance card (if you are using some form of insurance to help in paying for your visits)
  • your New Patient Packet (if you printed and filled in a packet ahead of time)
  • a list of your current medications for our records
  • any other supporting documentation from your doctor (optional)

Evaluation: Now that we have all your information, you will begin the initial evaluation process. This is a one on one private session with the physical therapist to test your current condition and determine what treatments will be needed to provide you with the best physical therapy. This evaluation process will help build your personal treatment plan, a customized list of treatments needed to return you to normal function.

After the evaluation process, often the patient is asked to begin their first physical therapy treatment session. If you have a limited time for your first visit, please let the therapist know to schedule your next appointment to begin treatment. Expect the information gathering, initial evaluation, and first treatment to take between 1 and 2 hours. The length of all visits after this will be dependent up on the individual's treatment plan and severity of problem.

What To Wear

During the initial evaluation and treatment process, it is helpful to wear loose, comfortable clothing and shoes. Wearing clothing you can easily exercise or move around in will make your visit more comfortable and provide the full range of motion needed for your treatment. Tight or restrictive clothing may cause problems during this process. Also be aware that your therapy may require use of the aquatherapy facilities. If you need aquatic therapy, please wear swim-appropriate attire such as a bathing suit or swim trunks. Towels and changing / locker room facilites will be provided at Paradigm for drying and storage during and after treatment.

Odessa Transportation

At our Odessa facility (only) we provide transportation for our current patients. If you are within our driving area, we can provide pick up and drop off service to your house. Our driving areas include the areas between the loop east and west across town, and the area between Interstate 20 and Yukon Street north to south. If you have questions about our coverage area, please contact the front desk in Odessa for more information. If you need bus service, please discuss it with the receptionist when you arrive for your initial evaluation on your first visit. At this time we cannot pick up patients for initial evaluations.
